Effective September 1, 2010 FSCO's Dispute Resolution Forms A and Chave been revised to reflect reforms to Ontario automobile insurance that will take effect September 1, 2010. Forms A, B, C and D have been amended to include a field where Claimants/Applicants have the option to include an email address.

The affected forms are:

• Application for Mediation (Form A) *
• Response to an Application for Mediation (Form B)
• Application for Arbitration (Form C) *
• Agreement to Neutral Evaluation at the Commission (Form D)

*The old forms will not be accepted after December 1, 2010
